Filing is a multi-step process. For each filing you must:

- Complete the conventional Withholding Tax coupon;
- Obtain a Certified Check from your bank for the amount of your tax payment.
- The Certified Check must be made payable to: New York State Department of Taxation and Finance. Include your Taxpayer ID Number on the memo line, on the face of the certified check. 
- Mail your coupon and payment to:
